Liability Waiver

I, the undersigned, acknowledge and agree that participating or observing in the fitness activities sponsored and offered by Premeditated Health and Performance carries inherent risks. These risks include, but are not limited to, extreme physical exertion, lifting weights, and other fitness activities. Specific risks encompass: 

- Injuries resulting from contact with other members, persons, or objects.

- Injuries stemming from interaction with fitness equipment.

- Injuries that occur due to negligence or insufficient training.

- Injuries, including those leading to death, arising from the misuse or negligent use of the facility or any equipment on the premises of Premeditated Health and Performance.

I am aware of these and numerous other inherent risks in observing or participating in the activities offered and sponsored by Premeditated Health and Performance. I ASSUME COMPLETE RESPONSIBILITY and liability for these risks and for any injuries that may arise as a result, EVEN IF such injuries occur in ways that are NOT FORESEEABLE at the time I sign this health performance waiver. By voluntarily assuming these risks, I understand that I will be SOLELY RESPONSIBLE for any loss or damage I sustain, including PERSONAL INJURIES to me, destruction of my property, or damages arising from my death.

WARNING: This agreement is legally binding. By signing it, I relinquish my right to seek compensation through the courts or otherwise for any personal injury, property damage, or death caused by negligence, accidents, or otherwise.

I recognize the potential risks and dangers involved in participating in activities at this location and waive and release Premeditated Health and Performance, including all staff, volunteers, landlords, and owners, from any liability related to the use of Premeditated Health and Performance.
